Request for Invitation Letter

In order to provide more accurate and timely service to our international attendees, CLEO/QELS/PhAST Management has initiated a new process for issuing invitation letters. You need to complete and submit the form below. Following completion of the online form you will receive an electronic Letter of Invitation within 7-10 business days. This service will be available until April 20, 2008. Unfortunately, we can no longer send the completed document to you by FedEx, fax or other means. Please note, CLEO/QELS/PhAST Conference Management is not able to contact U.S. Embassies on behalf of any individual attempting to gain entry into the United States to attend the CLEO/QELS/PhAST Conference.
