Please note: Each Short Course requires a separate fee and ticket. Seating is limited and advance registration is recommended. There will be no wait list for sold out Courses, and the Course Notes are not available for purchase as a separate product.
Registration
Registrations received after 23:59 EST (04.59 GMT next day) on Saturday, 31 December 2011 or 23:59 EDT (03.59 GMT next day), on Monday, 19 March 2012 will be charged the advance registration or on-site registration rates as applicable. Payment MUST be received with your registration form; NO forms will be processed without payment. All payments must be in US dollars and drawn on a US bank.
Confirmations
Upon receipt of your registration and payment, a confirmation letter will be e-mailed, faxed or mailed (based on your method of registration) to each registrant within 3 days. PAYMENT PROBLEMS WILL DELAY PROCESSING OF YOUR REGISTRATION.
Badge Credentials
Your registration confirmation will include a barcode which you will scan on-site at a Quick Registration Station in order to print your badge. Should you forget to bring the barcode with you, your badge can be obtained at the Pre-Registration Counters. Please Note: NO BADGES WILL BE MAILED.
**Your technical badge represents value. As such, all technical badges will be replaced for a $25 badge reprint fee.
Reasonable registration policies and terms have been established for CLEO: 2012. We encourage you to read these carefully as all registrants to CLEO: 2012 will be required to agree to these policies and terms as a condition of registration.
Privacy Policy
CLEO and its sponsors respect the privacy rights of our online visitors and recognizes the importance of protecting all information that you may choose to share with us. To further this commitment, we have adopted this Privacy Policy to guide how we collect, store and use the information you provide to us.
Payment
Credit cards are accepted. Payments made by check or money order must be made payable to the Optical Society of America in US dollars and drawn on a US bank. Payment must accompany your registration form in order to process your badge/ticket order.
Refund and Cancellation Policy
All requests for refunds must be received in writing by Friday, 27 April 2012. A US $75 service charge will be assessed for processing refunds. All registrations received after Friday, 27 April 2012, are NON-REFUNDABLE.
